A surgeon should choose All-on-4 versus All-on-6 based on patient-specific anatomy, available bone, implant stability, restorative design, biomechanics, opposing dentition, parafunction, and the desired distribution of support. All-on-4 can be predictable in an appropriately selected case, particularly when two anterior implants and two strategically tilted posterior implants can create adequate support and anterior-posterior spread. All-on-6 may be considered when anatomy and restorative space permit additional implants and the treatment team believes that greater distribution, redundancy, or prosthetic support would benefit the case.
The decision should not be reduced to the idea that six implants are always better than four. Additional implants can create surgical and restorative challenges if they are poorly positioned, too close together, or difficult to incorporate into the prosthesis. Conversely, relying on four implants may be less desirable when bone quality is limited, expected loading is high, implant distribution is compromised, or the treatment plan would benefit from additional support.
CBCT analysis and prosthetically driven planning are essential. The clinician should evaluate sinus and nerve location, arch form, ridge dimensions, implant length and diameter, restorative space, anticipated cantilever, primary stability, and whether the final prosthesis can be designed for passive fit, strength, cleansability, and maintenance.
