The number of implants needed for a full-arch restoration is determined by the patient’s anatomy, bone quality, restorative design, biomechanics, opposing dentition, available anterior-posterior spread, and the clinician’s ability to achieve stable and appropriately distributed support. Four implants may be sufficient in a properly selected All-on-4 case, while other patients may benefit from five, six, or more implants. The correct number should come from diagnosis and treatment planning rather than from a fixed rule.
A doctor should evaluate whether the planned implant distribution can support the prosthesis without excessive cantilever or concentration of force. Bone volume and quality, implant length and diameter, arch form, parafunction, restorative material, hygiene access, and the possibility of losing one implant should all be considered. In some cases, placing additional implants may improve support or provide greater flexibility. In other cases, anatomy may limit the number and require strategic use of tilted implants or an alternative treatment approach.
